Definitions of Change Management and Change Facilitation  (Germany)

I would like to collect some definitions of Change Management and Change Facilitation, and maybe eventually we can come up with a common language.

In my previous courses, I used the following one:

Change Management: the process, tools and techniques to manage the people-side of change processes, to achieve the required outcomes, and to realize the change effectively within individuals, teams, and the wider systems.

I found some more in the Internet:

Change management is a systematic approach to dealing with change, both from the perspective of an organization and on the individual level.

Fred Nichols, in his famous article "Change Management 101: A Primer" (probably the oldest text on Change Management in the Internet) gives four definitions:
1. The task of managing change (from a reactive or a proactive posture)
2. 
An area of professional practice (with considerable variation in competency and skill levels among practitioners)
3. 
A body of knowledge (consisting of models, methods, techniques, and other tools)
4. 
A control mechanism (consisting of requirements, standards, processes and procedures).

I could not find any definition of Change Facilitation. A field we could explore more?

This is one that I find simple, concise, and something that both management and employees can understand easily.

Change management is a systematic approach used to manage the people side of change, to achieve the expected business outcome.
